Please schedule events and childcare a minimum of two weeks ahead of the event date. Our forms are available to view and print as PDF files using the free Adobe Acrobat Reader. Completed forms may be faxed to (719) 548-9597 or submitted by e-mail.

You may continue to print out and fill in the form by hand, and fax it to the church. Or you may use our new interactive PDF form, which requires Adobe Acrobat Reader Version 7.0 or above (Vista requires Adobe Reader 9.0 or above).  This interactive method will allow you to fill in the form online. You can type inside text boxes or click to select check boxes. Use tab to move between fields. Click the Submit Button at the end to e-mail the file. Before closing, you may print and save the file if desired. If you have problems with the submit function, you should still be able to print your completed form and fax it.

Effective 12/10/08, we will reply to confirm receipt of all online Facilities Requests.
If you DO NOT get a response from Cheryl, please call the church office at 719-598-2139.

Facilities Request Form

(due at least 2 weeks before the event)

Childcare Request Form  (see below for additional information)

Kitchen/Fellowship Hall Cleanup Checklist  (fill out after your event if you are using these rooms)

Maintenance Request Form  (let us know about areas that need attention)

Troubleshooting:
As with all things new, there may be a few problems. If the Submit button doesn't work, you should still be able to print and fax the form. If you do want to upgrade to a new version of Adobe Acrobat Reader, I suggest the following steps.

  1. Go to Start / Control Panel / Add or Remove Programs, and remove your existing Adobe Reader.
  2. Reboot your computer.
  3. Go to www.adobe.com/products/acrobat/readstep2.html and follow the prompts to download the latest version. Uncheck " Free Google Toolbar".  You also don't need Adobe Manager or Photoshop..
  4. Reboot your computer again, and you should be all set.

Facilities Requests are presented during Staff Meeting on Tuesday morning and you may call to confirm your approval following that meeting.

Church Childcare Policy:
All requests for church provided childcare must be submitted to the church office at least 2 weeks in advance of the event. Greg Teel, Minister of Childhood Education, will need the names, ages, parents’ names, and allergies for each child no later than one week in advance of the event.

All events, other than ongoing scheduled church activities, will require a minimum of three children.  Please feed your child before or after the event unless prior approval is given by the Minister of Childhood Education. No evening events will have childcare after 8:30 p.m. and we do not provide childcare for children over the age of 11.
